  • Where can I find the reference number?

    The reference number is available on the 'Thank You' page when you submit your application online, as well as on the email or letter you have received from us.

  • Are there other ways to send in my documents?

    The easiest way to submit the required supporting documents is by using the Document Upload website. You may also submit your documents via post using the information below.
    Remember to write your reference number on all your documents. Please note that submitted documentation cannot be returned to you.

  • How can I reduce the size of the file I am uploading?

    You can upload files up to 10MB in size. Larger files will take more time to upload. To reduce the size of files, you can:
    • Scan or create documents in gray scale instead of color
    • Reduce the dots per inch (dpi) of a scanned image; 200 dpi should be large enough for your image to be readable

  • What is the maximum file size allowed for upload?

    The maximum size for a single file is 10MB. The maximum size for a submission is 30MB. Please note that larger files will take longer to upload.

  • What file types can be uploaded?

    The following file types can be : BMP, GIF, JPEG, JPG, PDF, PNG and TIF. Please make sure files are saved with the correct file extension.

  • Who can access the site to upload documents to American Express?

    Depending on the status of your Card application, you will be provided access to this site. You may submit the required supporting documents to American Express by uploading them via this site.

  • What if I need technical assistance uploading the documents?

    The answers to most common questions can be found within these FAQs. Please read them carefully in order to quickly resolve any issues you may encounter. If you are still having difficulties, please call us at the number shown on the email or letter you have received from us.

  • How can I make sure my files were uploaded?

    Please be sure to click the ‘FINISH’ button after attaching all your files. You will be directed to a confirmation page once your files have been successfully uploaded. Files that are not submitted will be lost upon closing the browser window and you will have to upload any file(s) again.

Below are the requirements for each document. Please confirm the requirements before proceeding to upload the documents. If you have any questions, please note your reference number and contact us by telephone at the number on the email or letter you received.

  • IDENTITY VERIFICATION - Please provide two of following official identification documents

    Driver’s License
    • Must be currently valid
    • Must include Address, Name and Date of Birth
    • Must be issued by National Public Safety Commission with the exception of International Driver’s Licenses
    • If there have been any updated details such as a change of address on the back of the license, an image of the back is also required
    • If the permanent legal address is different from the current residential address, please cross out the legal address. If the permanent legal address is the same as the current residential address, and the same as in the address field, please DO NOT cross out the legal address
    • Please cross out any insurance and medical information
    • Please DO NOT cross out other recorded details
    • Cannot be accepted if the ‘National Public Safety Commission Seal’ is not legible
    Health Insurance Cards
    • Must be currently valid
    • Must include Address, Name and Date of Birth
    • Issuer’s Seal on the copy must be legible
    • If the address is on the back of the card, an image of the back is also required.
    • Please provide the Health Insurance Card that shows your current address
    • Please provide identification documents which do not contain the ’Basic Pension Number/My Number/Health Insurance Number (except Long term care Insurance)/QR code’.
    Basic Resident Register Card
    *Photo card only
    • Must be currently valid
    • Must be a photo card
    • Must include Address, Name and Date of Birth
    • If there have been any updated details such as a change of address on the back of the card, an image of the back is also required
    Passport
    • Must be currently valid
    • Photo and holder’s details page (holder’s input column) are required
    • Please provide the Passport that shows your current address
    • Passports issued after February 4, 2020 cannot be accepted as identity verification documents as there is no "current address entry field"
    Residence Card
    • Must be currently valid
    • Must include Address, Name and Date of Birth
    • If there have been any updated details such as a change of address on the back of the card, an image of the back is also required
    • The number on the front must be legible
    Residence Certificate
    • Within 6 months from the issue date and does not contain ‘My Number’
    • If the ‘Issue Date’ and/or ‘Issue Seal’ are on the back, an image of the back is also required
    • Cannot be accepted if ‘Issue Date’ and/or ‘Issue Seal’ is not legible
    My Number Card
    • Must be currently valid
    • Only name, address and date of birth is required
    Special Permanent Resident Certificate
    • Must be currently valid
    • If there have been any updated details, such as a change of address on the back of the card, an image of the back is also required

    Certified copy of Commercial Registration
    • Must have been issued within the last 6 months
    • Must contain the Corporate Name, Representative’s Name, and Address of head office or main office
    • Cannot be accepted if ‘Issue Date’ and/or ‘Issue Seal’ is not legible
    Certificate of Registration
    • Must have been issued within the last 6 months
    • Must contain the Representative’s Name, the Corporate Name and Address of head office or main office
    • Cannot be accepted if the ‘Issue Date’ and/or ‘Issue Seal’ is not legible
